Palliative Care

Equipping people to cope with serious illnesses in the comfort of your home

Empowering Patients & Improving Quality of Life

What is Palliative Care?

Palliative care is a comprehensive care program focused on providing relief from the symptoms and stress of serious illnesses. Our Palliative team, made of specially-trained doctors, NPs, nurses, and other disciplines- can help you and your loved ones navigate and cope with the difficult experience. Our best-in-class palliative care is integral in providing comfort in difficult situations such as extreme or end-of-life illnesses.

Palliative Care vs. Hospice Care

Palliative care is very different from Hospice Care: it’s a program for a much more broad range of patients. It is based on the needs of the patient, regardless of your age, stage, or prognosis. Palliative care or palliative home care can be a great additional layer upon your primary treatment plan so you are as supported as possible in your treatment, comfort, and quality of life.

Our Palliative C.A.R.E. Approach

Together with your primary health care provider, our team coordinates a plan of care that will offer effective pain and symptom management in every part of your treatment.

Our team is available to help 24 hours a day, 7 days a week, both by telephone and through home visits. You can request a nurse, a social worker or a chaplain to visit you at home.

We care about your well-being and will work as hard as possible to ensure a quick response. You will receive visits from your team routinely and when you need them.

Discussions about treatment, choices like symptom/pain management, will be provided so that you understand your condition, care options and other needs.

We're going above and beyond.

Feedback from our patients and their loved ones not only motivates us to keep working hard, but it also speaks volumes about our successes. See what others have to say about Charter!
