How do I get Palliative Care?
Speak to your Primary Care Physician about initiating a referral to Charter Palliative Care. We will come to your home and talk about how we can assist you in addressing your symptoms, and develop a plan of care that will ensure relief from pain and other symptoms the best possible way.
Do I have to pay for Palliative Care?
This program is covered under many insurances. Refer to your plan benefit guide or contact us directly to find out if Charter Palliative Care is covered by your insurance.
Together with your primary health care provider, our team coordinates a plan of care that will offer effective pain and symptom management in every part of your treatment.
Our team is available to help 24 hours a day
7 days a week both by telephone and through home visits.
You can request a nurse,
a social worker or a chaplain to visit you at home. .
We care about your well-being and will work hard to ensure you receive a quick response. You will receive visits from your team routinely and when you need them.
